Alfaretta Adult Transition Program

All students who receive special education services are entitled to transition services, i.e., services that will prepare them for life after high school. As your student grows older, their IEP will become more focused on supporting them in making this transition, whether that means accessing higher education opportunities, career pathways, and/or other meaningful community engagement.

Beginning in the year your student will turn 16, their IEP must contain a transition plan that identifies their postsecondary goals and services that will help them attain these goals. If the IEP team agrees it is necessary, a transition plan can be added to your student’s IEP prior to this. In this plan, you will see how the school is intending to support your student throughout their time in high school, as well as the instruction, courses, experiences, and opportunities that will be provided to them to help them reach their goals.

For some students who present more significant needs, the IEP team may agree that participating in the district’s transition program, Alfaretta, is appropriate. Students may access this program through the year of their 22nd birthday. In this program, students receive vocational training, independent living skills instruction, and coaching on accessing postsecondary education, all supported by collaboration between the district and outside agencies. Alfaretta offers students a robust set of coordinated services to promote post-school engagement and help students live the lives they want once they leave Clover Park.
